- We bill Medicare, Medicaid and most private insurances
PRICELIST
We bill your insurance, so you are responsible for the deductible and amount not covered by your insurance, Medicare or Medicaid.
Registration forms must be filled out prior to visit and please have your medication bottles, names of other medical providers, insurance info, and pharmacy information ready at your visit time.
Please allow a 30 minute window to allow for traffic, we will do our best to communicate with you if we are late or early.
